Benefit system is a flexible, easy to use and time saving tool that allows you to:
- Check claim status
- View claim history
- View eligibility and benefits
- Access a copy of an Explanation of Benefits (EOB)
Provider Inquiry Form - To submit an inquiry request or send additional information to FAI regarding a specific claim, complete and return the Provider Inquiry form.
