Web-Based Application Access Instructions
Please complete the steps below to gain access to the FirstView Online Benefit System.
Step 1: Web-Based Application Access Agreement
Print, complete and sign two originals of the Web-Based Application Access Agreement. The agreements will be signed by a representative of First Administrators, Inc. and one original will be returned to your organization.
Note: Access will be granted only to the TIN(s) listed on this agreement.
Step 2: Terms and Conditions Agreement
Print and review the Terms and Conditions document and indicate your acceptance.
Step 3: Designate Individuals Authorized to Access the FirstView Online Benefit System
Complete the Authorized Representative Designation Form, listing individuals within the organization that are authorized to access the FirstView Online Benefit System.
Step 4: Send Documents to First Administrators, Inc.
Please mail or fax the two signed originals of the Web-Based Application Access Agreement and one original of both the Terms and Conditions and Authorized Representative Designation Form to the following:

Note: Photo copies will not be accepted. The Web-Based Application Access Agreements will be signed by a representative of First Administrators, Inc. and one original will be returned to your organization.
Step 5: Accessing the FirstView Online Benefit System
Once the Web-Based Application, Terms and Conditions and Authorized Representative Designation Form are received, the designated security coordinator will receive access information for all authorized users.
