FAI provides
the following:
- Certificate of Creditable Coverage to plan
participants when they terminate coverage and
when they
terminate COBRA coverage (if elected). The certificate
will also be provided upon request to active
Plan participants for up to 24 months after termination.
- Letter to the Plan participant upon receipt of an enrollment
form and/or Certificate of
Creditable
Coverage. This letter will outline any remaining pre-existing
condition period once the creditable coverage from the
former employer is applied.
